Digital Order Label
If you’ve placed an order online, you’ll need to select your Country, State, City, and the specific Chipotle restaurant you’ve visited. You can then enter the date and time of your visit and, when verified, you’ll have access to the survey.
Survey Link Via Email
If you’ve received a link to the survey via email, select the date and time of your visit as well as your Country, State, City, and the specific Chipotle restaurant you’ve visited.
